Welcome to The Avenue Surgery. We are committed to providing high-quality healthcare to our local community in Ealing. This leaflet is designed to help you make the most of our services, from booking your first appointment to managing your repeat prescriptions online. Our dedicated team of doctors, nurses, and support staff is here to support your health and wellbeing.

  • Hours: Monday to Friday, 08:00 – 18:30. 
  • Availability: Our phones remain on for the entire day until closing time. 

Our Practice Team  

We have a diverse clinical and administrative team to support your needs: 

Management & Clinical Leadership 

  • Management: Practice Manager: Sangeeta Kathuria and Deputy Manager: Carol Walters.
  • General Practitioners: Dr. Jane Livingston, Dr. Andrew Dharman, Dr. Anna Paul, Dr. Tamsin Robinson, Dr. Sarah Kayat, and Dr. Anastasia Shulkin. 
  • Clinical Pharmacists: Kelechi and Dyma. They are available on Mondays, Wednesdays, and Thursdays. 

Nursing & Patient Support 

  • Nursing Team: Practice Nurses Melanie and Venetta (Practice Nurse/ANP). 
  • Healthcare Assistant (HCA): Lorraine. 
  • GP Assistant: Jackie. 

Administration & Reception 

  • Administrators: Asmat, Jaideep, and Cristian. 
  • Receptionists: Ceri, Dulce, Fartun, Margaret, Linda and Jada. 

Appointments & Services 

  • eConsult: eConsult is free to use for NHS patients. Use it to ask about non-urgent health symptoms, conditions, or treatment, and even request sick notes or GP letters. Fill out a quick form online, and the practice responds with advice, a prescription, or an appointment, usually by the end of the next working day or sooner. 
  • Booking: You can book an appointment by calling reception on 020 8997 2525. Standard appointments are 10 minutes. We offer Video, Telephone, and face-to-face slots. 
  • Interpreters: Interpreters and BSL (British Sign Language) services are available. Please mention these requirements when booking your appointment. 
  • Double Appointments: Please book a double slot if you have multiple concerns to discuss. 
  • Registration: Residents in W13, W5, or W7 can register. Registration is done online only unless there is a valid reason you cannot obtain access. 
  • Home Visits: Reserved for housebound, terminally ill, or Nursing Home patients. A doctor will decide if a visit is appropriate. 
  • Cancellations: We rely on patients to cancel if they cannot attend. Repeated failure to cancel may result in being asked to register elsewhere. 

Non-NHS Work (Private Services)  

The NHS does not cover all services provided at this practice, and fees may apply in accordance with BMA guidance. For inquiries regarding these services, contact 0208 997 4661.

Online Access & Results 

  • NHS App: We encourage all patients to sign up for online access via the NHS App or AIRMID App. 
  • Benefits: Use these to view records, access results, and request prescriptions online. 
  • Test Results: Please check results online. You will be contacted if results are abnormal. Only call for results if you cannot use the online system. 

Repeat Prescriptions  

  • Requests: Must be in writing via email or the online clinical system. 
  • Notice: 48 hours’ notice is required for a repeat prescription to be prepared. 
  • Policy: Telephone requests cannot be accepted. 
  • For any new patients, you will need to book a medication review with our clinical pharmacist. 

Out of Hours Care  

  • NHS 111: For urgent problems when we are closed, call 111 (free from landlines and mobiles). 
  • Ealing Urgent Care Centre: Open 24/7 at 020 8967 5313. 
  • Ealing HUBS: Appointments available at Elm Bank Surgery out of normal hours and on Saturdays. 

Practice Policies 

  • Confidentiality: Your information is kept secure and only shared when there is a genuine clinical need. We are registered under the Data Protection Act. 
  • Zero Tolerance: Abusive behaviour toward staff or patients will not be tolerated and may result in immediate removal. 
  • Complaints: Please speak or write to the Practice Manager, Sangeeta Kathuria, for our policy. 
  • Disabled Access: Wheelchair access and a downstairs toilet are available on the ground floor. Please notify us in advance if assistance is required.
